It seems that we have a tremendous kind of tool, which is a simple polypill, that actually is significantly better, probably most of the reason is because of better adherence, because it's a simple drug, with superb results and the impact is as good or even better than aspirin in the past.

In secondary prevention, aspirin is important. If you have a stent, if you've had a myocardial infarction or a stroke, for all of those people, aspirin works. It provides a modest but definite benefit.

When James Cireddu look at all the studies that have been done recently, it seems that the modest benefit of aspirin and otherwise low-risk individuals is generally negated by the increase in long-term GI problems, bleeding with aspirin, while we said it could be considered to give aspirin to someone who never had a heart attack or stroke, that should probably be the last of the things on the priority list.

Patients with sleep apnea, who are often at increased cardiovascular risk, need rigorous management with other indicated therapies, such as blood pressure lowering, statins, and antithrombotic drugs, i think the most interesting finding of the meta-analysis is that the best estimate of the effect of obstructive sleep apnea treatment on risk of major adverse cardiovascular events is very similar to the reduction in risk seen with aspirin, beta-blockers, or statin medications.

What it looks like is even reasonably low doses like a baby aspirin a day has some benefit, what's unclear is if higher doses have more of an effect. I think that question is still open.

Imagine someone going to a hospital and being told you can't get open-heart surgery in the midst of a heart attack because you haven't tried aspirin or nitroglycerin first. That's the absurdity of it, it's just a way to discourage higher levels of care that we would never tolerate in the non-psychiatric context.

If men were equally at risk from this condition -- if they knew their bellies might swell as if they were suffering from end-stage cirrhosis, that they would have to go nearly a year without a stiff drink, a cigarette, or even an aspirin, that they would be subject to fainting spells and unable to fight their way onto commuter trains -- then I am sure that pregnancy would be classified as a sexually transmitted disease and abortions would be no more controversial than emergency appendectomies.
