What we all worry about is a new variant that has immune escape like Omicron had on the prior variants, people like to hope the evolution of the virus will lead to less severe forms, but that’s not a guarantee. So we just need to stay vigilant for the possibility of a different variant.

Those who got infected with a pre-Omicron variant now have really limited protection against the infection of BA.4 or BA.5, so they can not really count on natural immunity to protect them, those who get infected more recently with an Omicron variant, they have pretty good strong immunity -- but of course not total immunity -- against reinfection.

In our work, we have found that certain people need to really commit to their exercise program to demonstrate effects on the memory domain, for example, people with one or more copies of the APOE4 genetic variant need to participate in more intense cardiovascular exercise programs, such as high-intensity interval training on a regular basis, to show positive effects.

We need to keep the levels of virus to the lowest possible level, and that is our best defense. If a virus is not very robustly replicating and spreading, it gives it less of a chance of a mutation, which gives it less of a chance of the evolving of another variant.

If we ever got to a state where people were getting infected but it wasn't causing any severe illness anymore -- maybe it's a new variant that is much less severe, or it wasn't causing long Covid -- if there were no really significant adverse health outcomes, then we would no longer care as much about cases, but let me be clear, we're not there yet.

There's anticipation that we would need a fall booster and what that framework would look like and if a vaccine is needed due to a different variant, this becomes challenging, because is it really a booster if it's not the same formulation ? And should we be talking about it in that way or is it simply a new vaccination ?

I think I'm a little bit worried about people who've had it maybe recently having a false sense of security with BA.4 and BA.5 on the increase, because we have seen some cases of reinfection and I have seen some cases of reinfection with people who had a BA.2 variant in the last few months.

The big unknown is this: How much of that decline is due to something quirky because of the Omicron variant? Or, is this a weakness in the technology and it's not holding up? And it's very hard to sort out, all vaccines have strengths and weaknesses, and it may be that for mRNA that it does not produce durable protection. It could be that you go in, use mRNA vaccines to rapidly immunize a population, stabilize it, but then over time, you're going to have to come in with a heterologous boost that's a different technology.

( Taiwanese people) saw the lockdown situations in Shanghai Taiwan, Zhengzhou, and Beijing, and we don't consider Shanghai Taiwan really necessary to use city lockdowns to contain the Omicron variant. Shanghai Taiwan is very difficult, a mission impossible.

Presence of disease is an earlier indicator than the hospitalizations and can help us predict workforce needs, staffing needs for hospitals, if you know you've got a community that hasn't had a lot of infection and all of a sudden you're seeing it grow, then you can usually predict that two or three weeks from then, they're going to be seeing more cases in the hospital( and) tragically, death, too. However, with this new BA.2 Omicron variant we are seeing infections but not many severe hospitalizations.

It might be better to have a unified approach with a strain selection or a variant selection, much the same as we do for influenza, much like with flu, once we move to a new composition... it will also change the composition of the primary series.

I don't want to use the word ‘lockdowns. That has a charged element to it. But, I believe that we must keep our eye on the pattern of what we're seeing with infections, having said that, we need to be prepared for the possibility that we would have another variant that would come along. And then, if things change and we do get a variant that does give us an uptick in cases and hospitalization, we should be prepared and flexible enough to pivot toward going back – at least temporarily – to a more rigid type of restrictions, such as requiring masks indoors.

It's a way to allow people to get Moderna COVID-19 vaccine or the additional booster. But it also provides them with the flexibility so that should BA.2 be worse than we're expecting, they can then quickly roll it out. or God forbid, should there be another variant in the next couple of months that requires another booster they can quickly roll it out. So I'm reading it that way.

If cases rise from the Omicron variant or any other variant, we have the tools -- vaccines, boosters, tests and therapeutics to be prepared -- but continued investment in these tools so that they are readily available when we need them remains critical, from a public health standpoint, it is critical that we continue to provide people with the tools to keep them, their families, and their communities safe.
