Pilot error
Pilot error is a decision, action or inaction by a pilot of an aircraft determined to be a cause or contributing factor in an accident or incident. Pilot error can be a mistake, oversight, lapse in judgement, lack of training, loss of situational awareness, or failure to exercise due diligence by pilots during the performance of their duties. A broader view of how "human factors" fit into a system is now considered standard practice by accident investigators when examining the chain of events that led to an accident.
