Wikipedia

  1. Fentanyl

Fentanyl, also spelled fentanil, is a highly potent synthetic opioid used as an analgesic. Together with other drugs, fentanyl is used for anesthesia. It is also used recreationally, sometimes mixed with heroin, cocaine, benzodiazepines or methamphetamine. An overdose of it can be neutralized by naloxone. Fentanyl is sometimes used as an additive in counterfeit pills disguised as legitimate medication. It has a rapid onset and its effects generally last under two hours. Medically, it is used by injection, nasal spray, or transdermal patch.Common adverse effects of fentanyl include nausea, vomiting, constipation, itching, sedation, confusion, and injuries related to poor coordination. Serious adverse effects may include hypoventilation, hallucinations, serotonin syndrome, low blood pressure, or development of an opioid use disorder. Fentanyl works by activating mu-opioid receptors. It is around 100 times stronger than morphine and about 50 times stronger than heroin. Fentanyl was first synthesized by Paul Janssen in 1960 and approved for medical use in the United States in 1968. In 2015, 1,600 kilograms (3,500 pounds) were used in healthcare globally. As of 2017, fentanyl was the most widely used synthetic opioid in medicine; in 2019, it was the 278th most commonly prescribed medication in the United States, with more than a million prescriptions. It is on the World Health Organization's List of Essential Medicines. In 2021, fentanyl and fentanyl analogues accounted for most drug overdose deaths in the United States with 71,238 deaths. Compared with heroin, it is more potent, has higher profit margins, and, because it is compact, has simpler logistics. It can cut into, or even replace entirely, the supply of heroin and other opiates. The flow of fentanyl mainly originates in Chinese factories which produce fentanyl or fentanyl precursors; it is then trafficked to other countries for illicit production and sale. In the United States, finished fentanyl arrives primarily from Mexico smuggled by cartels. In 2022, the US Drug Enforcement Administration (DEA) intercepted and seized more than 10,000 lb (4,500 kg) of fentanyl and over 50 million fentanyl pills, more than double what was seized in 2021 and enough to kill every person in the United States, nearly 330 million.

  1. Fentanyl

Fentanyl is a potent, synthetic opioid analgesic with a rapid onset and short duration of action. It is a strong agonist at the μ-opioid receptors. Historically it has been used to treat breakthrough pain and is commonly used in pre-procedures as a pain reliever as well as an anesthetic in combination with a benzodiazepine. Fentanyl is approximately 100 times more potent than morphine, with 100 micrograms of fentanyl approximately equivalent to 10 mg of morphine and 75 mg of pethidine in analgesic activity. It has an LD50 of 3.1 milligrams per kilogram in rats, and an LD50 of 0.03 milligrams per kilogram in monkeys. Fentanyl was first synthesized by Paul Janssen in 1960 following the medical inception of pethidine several years earlier. Janssen developed fentanyl by assaying analogues of the structurally related drug pethidine for opioid activity. The widespread use of fentanyl triggered the production of fentanyl citrate, which entered the clinical practice as a general anaesthetic under the trade name Sublimaze in the 1960s. Following this, many other fentanyl analogues were developed and introduced into medical practice, including sufentanil, alfentanil, remifentanil, and lofentanil.
